This seminar covers all of the elements of a problem/condition reporting, root cause analysis and corrective action system with emphasis on the following root cause analysis techniques: Failure Mode and Effects Analysis for hardware problems and Hazard-Barrier-Effects Analysis for management and technical process problems. This seminar will also cover a modified Hazard-Barrier-Effects Analysis technique that allows the root cause analysis resource expenditure to be proportional to the significance of the problem, while still enabling the analyst to identify human performance root causes.
